You can remove consent forms from a case if they are not signed or only have one signature. You cannot remove a consent form from a case if it has both signatures.
To remove a consent form
1. In the Treatment Plan Case Setup section of the Navigation panel, select the case that you want to delete a consent form from.
2. In the Treatment Plan Case Setup section toolbar, click Consent Forms.
The Dentrix Consent Forms dialog box appears.
3. Select the consent form that you want to remove.
4. To remove the form from the case, click Remove Selected Form, and then click Close.
